Cremation forms and fees

Cremation can only proceed once the cause of death is confirmed.

Your funeral director will:

• liaise with Penmount on your behalf to book a date and time for the funeral
• assist with funeral arrangements
• ensure all the necessary forms are submitted

Forms

All of the required forms will be completed with your funeral director.  They include:

Preliminary application form

• Initial request for cremation

Cremation form 1

• Signed by the next of kin or executor
• Includes details of any medical devices that may need removal

Cremation form 3

• Application for cremation of a stillborn baby

Green form (Certificate for Burial or Cremation)

• Issued by the registrar
• Not required if the coroner provides Cremation Form 6 instead

Cremation form 10

• Signed by the Medical Referee at the crematorium

All completed forms must be received no later than 09:30, two working days before the cremation date.

Prices effective from 1st April 2026

Please note the cremation fee includes:

• environmental levy (if applicable)
• cremation fee
• medical referee fee
• use of organ and music system
• container for remains
• scattering of remains in the Garden of Remembrance
