This form is used solely for the purposes of processing your claim for compensation and can be used for all claim types such as vehicle damage, personal injury or property damage. Cornwall Council does not accept this form as a notification of defect. To notify us of a defect on the highway, you must telephone 0300 1234 222.
Please provide as much information as possible so that we can investigate your
claim fully and fairly.
What you need to include
To prevent a delay in processing your claim please have ready to upload;
- The reference number of the reported defect - this will start with a W or a M
- Map marking the location
- Photographs of the defect and surrounding areas (if available)
- Photographs of the damage (if available)
- Independent estimates of the damage repair
Please check the size of your images and documents before attempting to upload to the form. Attached documents and images must not exceed 10mb in total.
Cornwall Council has a responsibility to its Council tax payers to ensure that all claims are assessed individually and fairly; Where the Council could be held legally liable compensation will be arranged in a timely manner.
The Council and its insurers have an anti-fraud system in place to assist them in detecting dishonest claims and taking appropriate action.
Please note: Claims without precise location information, date and time of incident cannot be investigated. Please enclose photos of the damage incurred and the defect where possible.
