Please complete this form to register your service with the Cornwall Family Information Service (FIS) directory.

Once your submission has been reviewed and approved, your service will be added to the directory. You will then receive a login so you can update and manage your listing at any time.

Safeguarding

The Council is committed to safeguarding and promoting the welfare of children. The Council expects all staff, suppliers and volunteers to share this commitment to:

  • proactively prevent abuse occurring and

  • respond if abuse is identified, suspected or disclosed.

By sending this form to the Family Information Service, you agree that:

  • you will report any worry about abuse, harm, or risk of harm straight away. This includes concerns about children, vulnerable adults, or someone’s behaviour.

  • for vulnerable adults, call the Safeguarding Adults Team on 0300 1234 131

  • for children, call the Multi-Agency Referral Unit (MARU) on 0300 123 1116

Keeping people safe:

You have clear steps in place to keep people safe. These follow the law and current Council and Government rules. You can show that:

  • all staff and volunteers who work closely with children or vulnerable adults have the right DBS checks.

  • you check these DBS records on a regular basis.

  • you do not employ anyone who is barred or who may be unsafe to work with others.

  • your safety policy encourages staff and volunteers to speak up if they are worried about abuse.

The information you provide is being collected by Together for Families for the purpose of processing your information request and will be retained for the period of the inquiry. We may share this information with other relevant professionals to inform their work. The data held relating to the delivery of support by Together for Families will be used for the provision of services. This information will be held in a secure environment in accordance with our retention policy after which time it will be destroyed in a secure manner.
