Family Help for children and young people with disabilities


Family Help supports children, young people and families when they need extra help. It works with families to understand their needs, build on their strengths and tackle challenges early. The aim is to help children stay safe, improve wellbeing and achieve positive outcomes.

Our Disabled Children and Therapy Service supports children and young people with additional needs and disabilities, and their families, by providing coordinated services that help children thrive at home, in education, and in their communities. They do this through the following:

Portage 

Portage is a home visiting educational service for children aged 6 months to 3 years with complex needs and disabilities and their families. 

Portage can help by working with families and supporting development through:

  • play and communication

  • celebrate progress

  • provide information about groups and services, and

  • coordinate with other professionals

Supporting Change in Partnership (SCIP) 

Supporting Change in Partnership is solution-focused service working with disabled children and young people aged 0 to 18 and their families. 

SCIP can help by providing practical, preventative support including:

  • behaviour

  • communication

  • anxiety

  • sensory, and

  • family support

Team Around the Child Meetings

A Team Around the Child (TAC) meeting is a coordinated planning meeting that brings together families and professionals supporting a child or young person. 

It works by:

  • creating shared plans

  • coordinating actions, and

  • regularly reviewing progress
