Privacy Notice - Young Futures Prevention Panel


This privacy notice tells you about what information we collect about you, how we use that information and who we share it with.

Why do we collect your personal information?

The information you provide will be used for the following reason(s): To provide and improve services; For safeguarding children or adults.

What personal information do we collect?

The referral form asks for personal information about you, your family, and challenges that you may be experiencing - this is to help the Panel to connect you with the best package of support. The information includes the following:

Personal Information:

Your name, address, email address, telephone numbers, date of birth. Your sex or gender, your gender identity, your ethnic background.

Languages you speak. Your religion or beliefs. Any cultural needs or requirements.

Health and Wellbeing:

Your mental health and emotional wellbeing. Any special educational needs or disabilities (SEND). Drug or alcohol-related concerns. Experiences of social exclusion or bullying. Concerns about hate-related incidents.

Family and Home Circumstances:

Whether you are a child in care (Looked After Child). Domestic abuse concerns within the home. Whether a parent or carer is in prison. Drug or alcohol-related issues within the home. Mental health concerns affecting parents or carers.

Safety and Risk Information:

Times when you have been reported missing. Involvement in anti-social behaviour. Concerns about risk of child exploitation. Concerns about carrying or using weapons. Concerns about any harmful sexual behaviour. Whether you have been a victim of crime. Links to adults or peers who are known to the police where this may be relevant to safeguarding.

What sensitive information do we collect?

We collect the following sensitive information:

  • Your sex or gender

  • Your gender identity

  • Your racial or ethnic origin

  • Your religious or philosophical beliefs

  • Your health information

More information may be needed depending on your individual circumstances

What is our legal basis for collecting and using your information?

We use the following legal reason for obtaining your information: Consent.

Consent is obtained by the referrer before a referral is submitted. This may be from you where appropriate, or from your parent or carer. Information about the referral process is provided to support informed consent. Details of who gave consent, when it was given, and a summary of the consent discussion are recorded on the referral form and in the relevant Council system.

Your consent remains valid throughout the period of support and for 12 weeks afterwards to allow the Panel to monitor outcomes.

You can withdraw your consent at any time. If you do so, information sharing will stop unless we have a legal duty or safeguarding reason to continue sharing information. The withdrawal of consent will be recorded on the relevant Council systems and relevant partners will be notified.

Who do we share your information with and why?

We may share your information with other organisations, as allowed by law, and partner agencies that provide services on our behalf.

Information is only shared with relevant Cornwall Council services and professionals involved in supporting you. Access is restricted to Young Futures Panel members, the Data Support Officer, and project leads who need the information to carry out their role

We will share your information with the organisations that are represented at the Panel which include:

  • Services in Cornwall Council

  • Devon and Cornwall Police

  • Cornwall Partnership NHS Foundation Trust (Children’s Mental Health Services), and

  • a Voluntary and Community Sector representative

There may be other organisations we need to share your information with; we will always make sure this is done in compliance with the law.

How do we keep your information secure?

The records we keep about you are secure and confidential within the Council. The Council has a range of procedures, polices and systems to ensure that access to your records is controlled appropriately. Anyone who receives information from us is also under a legal duty to only use the information for the purposes agreed and keep the information secure and confidential.

Where do we store your information?

Where necessary, we may transfer personal information outside of the UK. When doing so, we comply with the UK General Data Protection Regulation (GDPR), making sure appropriate safeguards are in place. Please contact us for more information.

How long do we keep your personal information?

After we deliver a service to you, we must keep your information as a business record of what was delivered.

Each organisation involved in the Young Futures Prevention Panel keeps its own records in line with its retention policy and legal requirements. Cornwall Council's Panel records will be kept in accordance with the Council's retention policy for safeguarding and community safety records. Support through the programme will usually last up to 12 weeks, although this may be extended with parental consent. Information about your involvement in the programme will be retained for up to 12 months to support re-referrals and evaluate the programme. Relevant safeguarding information will also be recorded in relevant Council systems and other appropriate case management systems. Once this information has been transferred, Panel records will not usually be kept for longer than necessary
