Child Health Checklist
The Child Health Checklist will help you decide if you want support from the School Nursing Team.
Use the school age links below to access the relevant checklist.

If you have any questions or concerns about your child’s health after completing the Child Health Checklist, ask the School Nursing Team for support or advice.
To make an appointment

You can print the Child Health Checklist. You do not have to complete it to ask for advice or an appointment.
Routine Health Assessments
When your child starts Reception, Year 6 and is in Year 9 they can have a routine health assessment.
To make an appointment

All other School Nurse Health Assessments
The School Nursing Team also offer a range of Health Assessments at other times between the ages 5-19 years.
