Capital Incident Reporting

Capital Projects - Incident data and hours worked

This form is to be completed and submitted by the 5th day of the month for all hours, audits and incident data for the previous month.

Please use this form to report on Capital incidents, including the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR). It is the responsibility of the contractor to report all accidents and incidents, where necessary, to the enforcing authorities.

Your contract with Cornwall Council requires that you submit this information which will be held by Cornwall Council's Economy, Regeneration and Assets Service (Sustainable Growth and Place Directorate). The information that you provide will be stored securely and kept in a database to monitor incidents reports across Capital Projects. Your information may be shared with internal staff involved with the named project and external staff, such as consultants, who are involved with the named project. In accordance with Cornwall Council’s Record Retention Policy, the information you provide on this form will be retained for a minimum of 5 years. All personal information held by Cornwall Council is held safely in a secure environment.
