Supporting young people leaving care
Workers are based all across Cornwall. We do not run a duty or drop-in service. What the team does:We work with young people who are currently in local authority care as they 'age out' of the care system....This looks like:preparing young people for the time when they are no longer in careproviding advice and assistance after they leave caresupporting them through the transition from being in care to independent livingimproving the life chances of young care leaversYoung People are transferred to the 16 Plus Team at 16.They are allocated a 16 Plus Social Worker who works with them until 18....They are also allocated their own personal advisor, who will work with them from 16 to 21, or 24 if in higher education. This is a different person to the Social Worker.

Young people leaving care
If you are or have been in the care of the authority, it is important to know about:
Your pathway plan

Finding somewhere to live
In care for less than 13 weeks
Further advice
Your pathway plan
You should have a pathway plan with your social worker that shows how you will move to independent living....Social services will pay them an allowance until you are 21 to provide you with ‘supported lodgings’....You are in priority need as a care leaver if you are 18, 19 or 20 and spent 24 hours or more in care while you were 16 or 17.
