Circumstances of absence

If you have selected:

Option A: Complete ‘Part 1’ only and submit the form. School should continue to offer support and monitor the situation.

Option B: Complete ‘Part 1’ only and submit the form. School may wish to liaise with their Cornwall Council Attendance Improvement Officer for further guidance or should the situation change.

Option C: Complete ‘Part 1’ & ‘Part 2’ and submit the form. Cornwall Council will then be in contact with the school.

NB: Provision of this information does not enact a Section 19 responsibility on the part
of Cornwall Council.

Privacy Statement

The information you provide is being collected by Education and Community Health, Together for Families Directorate for the purpose of investigating a child potentially missing education and will be retained up to their 25th birthday.

This information may also be shared with other professionals in conjunction with the nature of the request or enquiry.  The data held relating to the delivery of area will be used for the provision of this service.  This information will be held in a secure environment in accordance with the TFF data retention policy after which time it will be destroyed in a secure manner.  If you wish to see what data we hold about you, or ask we rectify this data or erase the data you should contact the Education Welfare Service.
