CHOICE outreach is a form of community support, like a personal assistant. Its purpose is to provide inclusion support. Our support workers can assist you from home or in the community. Please note, we cannot perform personal care from your household.

How to arrange this session
If you are: self-funding, using a direct payment, or an ISF holding organisation you can contact the provider directly to arrange this session.
