New application
If this is a new application, please telephone the Licensing Team on 01872 324210 to arrange a face to face appointment for an identity check, where you will be required to produce the following original hard copy documents:-
- DBS identification documents or DBS certificate if already obtained and you are registered with the DBS Update Service
- Proof of right to work in UK

Any other relevant documents not submitted online, can also be produced at the face to face appointment.
Please note your application cannot be progressed until after the appointment.
Renewal application
If this is a renewal application and you are registered with the DBS Update Service, you are not required to attend a face to face appointment. However, if you have not already done so, your original DBS certificate must be produced to us before we can carry out the DBS update check.
Variation application
If this is a variation application, you are not required to attend a face to face appointment.
Please note, for both New and Renewal applications, the original hard copy of your medical examination report (unless the Doctor or Surgery emails this to us direct) must be produced before your licence can be issued.
The medical form can follow separately if your medical appointment has not yet taken place at the time of your application.
