Our teams
We have a number of different teams across the service, providing countywide or regional services (West, Mid or East Cornwall), as well as a number of specialist and multi-disciplinary teams
Rapid Response Team
There is a rapid response team located West, Mid and East Cornwall to provide immediate response and intervention to a social care crisis where there is a risk of admission to a care setting or where a significant increase in current support is required.
Assessment Team
There are six teams, three working in the community, and three working in the community hospitals.
The community assessment team ensure new and returning customers receive a timely first assessment. The community hospital assessment team facilitate timely discharges from Community hospitals.
Care and Support for Adult Years Teams
There are three teams located in the West, Mid and East of Cornwall who provide identified social work interventions, ongoing care coordination, support and a point of contact for customers with eligible social care needs who receive on-going services from the Council.
Care and Support for Older Adult Years Teams
There are six teams located across the county who provide older adults with ongoing care coordination, support and a point of contact for customers with eligible social care needs who are in receipt of ongoing services provided by the Council.
Acute Hospital Social Work Team
This countywide team ensures customers in acute hospital settings receive a timely, and appropriate social needs assessment.
Deprivation of Liberty Safeguards Team (DOLS)
A countywide team, providing legal protection to vulnerable customers who are deprived of their liberty and Approved Mental Health Practitioners (AMHP’s).
Adult Safeguarding Service
This countywide service assesses referrals of abuse or neglect in relation to adults with care and support needs who may be unable to protect themselves. They team also leads in co-ordinating and making enquires where organisational abuse is suspected and provide advice and guidance where concerns are raised regarding a person working in a position of trust.
