Before being on the agenda for a meeting:
- individual cases must be discussed with the appropriate:
- Head of Service at Cornwall Council; or
- NHS Kernow Continuing Care Manager
- This will ensure that all the necessary information is available. It also ensures that appropriate actions have already been taken
- Casework management and funding systems must be followed before discussion of individual cases at this meeting
