Transition can be an anxious time for young people and their parents/carers. They may be unsure about what to expect and what help may be available in order to plan for the future. There may be uncertainty about the roles of different agencies and the support that can be given.

The Cornwall Multi Agency Transition group produced a multi-agency transition protocol in order to improve the transition process for young people.

The protocol sets out what should happen and when and who should take responsibility.

The individual transition plan reflects all aspects of the young person's transition to adult life and involves everyone that contributes to their care.
