Key Transition points include moving from: 

  • Class to class
  • Early years to Primary School
  • Primary School to Secondary School 
  • Secondary School to work, college or university

It is just as important to ensure a smooth transition from class to class as it is from key stage to key stage and setting to setting. For children and young people with Special Educational Needs or Disability (SEND), preparation for transition should be part of the ongoing Assess, Plan, Do, Review cycle.

This Transition Planning Guidance document provides an overview of the key principles of transition planning and gives useful ideas and resources to support pupils through times of transition. It also gives planning suggestions for four key transition points listed above.

Supporting Children and Young People with Special Educational Needs and Disabilities Transition Planning Guidance is a useful resource for both the feeder setting and the receiving educational setting.

One Page Profiles 

A One Page Profile, is a single page which gives a summary of an individual’s personal views about themselves to others. It may convey  strengths, views, wishes and support needs. At Transition, it can capture key information to assist in understanding and appreciating the views of the child or young person and how to support them.

NASEN Transition Guide. Another useful document which explains the transition process in detail. Covers a range of transitions. Aimed at schools and other educational settings.

Moving Up – A Guide for Parents. Produced by Hertsmere Leisure, this booklet explore common worries and questions parents and carers may have. It particularly focuses on the move from pre-school and nursery to primary school.

Moving On – Top Tips. A document aimed at children and young people with more complex needs. Colourful and informative. Aimed at children moving between primary and secondary school. For schools and parents to use with their child.

SEND Transition Journal. Created by Central Bedfordshire Council, this booklet is aimed at young people of 16 years and older who are making the transition to college. 

Everyone has a part to play in preparing children and young people for adulthood from the earliest stage. This involves raising the aspirations of children and young people and their parent/carers and supporting pupils to achieve the best possible outcomes in education, employment, independent living and participation in society.

High aspirations are crucial to success, so discussions about long term goals should start early and ideally well before Year 9 (age 13-14) at school. Discussions should focus on the child or young person’s strengths and capabilities and the outcomes they want to achieve.
