The foundations are being put in place for the delivery of more integrated health and social care service, with the development of the Shaping our Future Programme. Partners from across health, public health and the Council are all working together to develop plans to help people stay as healthy as possible, support people to remain independent and well in their community and make the system more sustainable by focusing on prevention.

We are also investigating how the work we are taking forward locally can be supported and enhanced by our devolution status by seeking freedoms and flexibilities, more control and ownership over health and care related budgets and possibly transition funding. 

Health and care organisations in Cornwall and the Isles of Scilly have published their proposals for service transformation over the next five years.

Preventing ill health and more care at home are at the heart of health and social care plans for Cornwall and the Isles of Scilly.

The local Sustainability and Transformation Plan (STP) is a response to the NHS England Five Year Forward View and Devolution Deal for Cornwall to achieve three aims:

  • Improve the health and wellbeing of the local population.
  • Improve the quality of local health and care services.
  • Deliver financial stability in the local health and care system.
