Guidance on Provider Quality Feedback


The Quality Assurance Team collates feedback about services from a variety of sources. These include: 

  • providers
  • social workers
  • community health teams

Any compliments, comments or concerns about a care service can be shared with us using the Provider Quality Feedback (PQF) form.

The form is a single point of contact for adult care providers to notify us of:

  • isolated incidents
  • quality concerns
  • compliments and comments

at their services. It is also for health and social care partners to feedback on the quality of care and support services for adults in Cornwall.

It is our expectation that issues, or individual complaints are raised with the manager of the appropriate adult care service in the first instance.

Safeguarding concerns where people are at risk or potential risk of harm should be reported via the safeguarding process

The form contains questions depending upon the following:

  • Is this a provider self notification?
    Yes if, you are an adult care service reporting an isolated incident in line with the Safeguarding Adults Local Policy

  • Do you work in safeguarding triage?
    Yes if, you work in the Safeguarding Adult Triage team and are sharing an adult safeguarding concern, linked to an Adult care service

  • Are you feeding back the outcome following a Section 42 Enquiry?
    Yes if, you are a Social Worker who has carried out a Section 42 enquiry linked to an adult care service, and you are feeding back the conclusion of the enquiry

  • Are you reporting concerns about the quality of a contracted service?
    Yes if, you are a health and social care professional partner and have concerns about the quality of the adult care service

  • Complimenting areas of good practice?
    Yes if, you are a health and social care practitioner / partner and have positive feedback about the quality of the adult care service
