Early Intervention Service

Local people have told us that they want their care services as close to their home as possible, and they would like health, social care and a range of other services that offer support and care, to work together better, to meet people’s needs and improve their wellbeing.

To achieve this, we integrated teams from Peninsula Community Health and Cornwall Council’s Adult Care and Support into six health and social care early intervention services (EIS), across Cornwall. These EIS services have close links to local GP practices, community groups and volunteers, so that support is available from as wide a range of organisations and people as possible.

The directory provides advice and information about support at home, leisure activities, getting around, health and wellbeing, work, learning and volunteering, advice, information and money matters, staying safe, caring for someone and housing and care homes. It also has a 'What's On' section where you can find details of events, activities and courses.
